The focus is on bringing about positive change for this vulnerable client group. The main areas of work have been around:

  • Developing a clear, concise and accessible patient passport.
Ensuring that pre-discharge meetings are arranged for patients with learning disabilities.
  • Training our staff involving users and carers.
  • Improving discharge information.

The strength of the project is its true partnership approach between the organisations involved. This has meant that the improvements made for patients have been sustained. View the latest trust report on this work.

We are always keen to hear about people's experiences of the service.  If you would like to become involved or make a comment, please email
