Macmillan Specialist Palliative Care Team

(adult services)

What do we do?

The Specialist Palliative Care Team is made up of health professionals who provide support and advice to individuals affected by cancer or other advanced incurable illnesses, and their carers. We work alongside other health and social care providers such as GP’s, hospital ward teams, community nurses, social workers and other therapists. The goal of palliative care is to promote quality of life for people living with life limiting illnesses.

Our services

  •                   Advice on the management of pain and other symptoms.
  •                   Psychological / emotional support for individuals and their carers.
  •                   Information on types of cancer and its treatment.
  •                   When help is needed to plan future care.
  •                   To provide direction to other appropriate health and social care services.
  •                   To provide information on financial help.

Within East Cheshire NHS Trust there are 2 Macmillan Specialist Palliative Care Teams, one covering Eastern Cheshire and the other covering South Cheshire and Vale Royal.

Where are we based?

The team is based at Macclesfield District General Hospital, although we work across the hospital site and the community to visit patients in their own homes or other places of care such as nursing and residential homes.

South Cheshire and Vale Royal Service

The Central Cheshire team provides Specialist Palliative Care for those patients registered to one of the 36 practices that are in central Cheshire; including Holmes Chapel, Alsager, Scholar Green, Rode Heath, Sandbach, Middlewich, Holmes Chapel. In the west some of the area served by the Tarporley and Audlem Practices that are beyond Vale Royal and Crewe and Nantwich are also served by the team.
