Community Business Group

Community services were integrated with hospital services in April 2011. The Community Business Group will focus on the proactive management of long term health conditions, elderly care and intermediate care services. Community care services are based in two localities: one covering practices in eastern Cheshire and the other covering south Cheshire and Vale Royal. This will enable district nurses, specialist nurses, community matrons, health visitors and therapies to be readily accessible and available to work alongside social workers, voluntary and statutory organisations and local GPs in identifying clinical priorities and meeting health and social care needs. 

The aim is to provide as much care out of hospital as possible, designing and improving services that build on work already happening in community and practice settings.  Consultant led outpatient services and therapy services will continue to be provided in a range of community settings across the central and eastern Cheshire footprint.
