The Complex Care Service is for children and young people and covers the Central and Eastern Cheshire PCT footprint caring for children and young adults requiring various types of clinical intervention. Bespoke care packages are allocated after an assessment has been completed, the child has met the criteria and funding has been agreed.
The aim of the service is to support children and young adults with long term complex health needs at home by providing care to optimise the highest quality of life and independence. The service also aims to treat children and young people in the community for as long as possible to avoid hospital admissions.
To support children and young adults with long term conditions who live at home. We aim to treat patients as long as possible without hospital admissions.
The team leaders manage the packages of care and the nurse trainers train and educate the health care assistants (HCA) using competencies and training tools. The HCAs when competent deliver care in the home environment supporting both the child and the family.
