Hospital Team
Hospital dietitians provide specialist dietary advice to help patients to manage their medical condition and maintain nutritional status while in hospital and to promote recovery. Dietitians work closely with multi disciplinary teams such as paediatrics, diabetes, critical care, gastroenterology, stroke and oncology, including specialist outpatient clinics.
GP Practice Team
GP Dietitians provide an outpatient service for patients to attend in their GP practice. Within these clinics the dietitian provides advice on therapeutic diets for disease management.
Home Enteral Feeding Team
This team provides oral nutrition support and tube feeding advice to patients in residential, nursing homes and their own homes.
Weight Management Team
The weight management team provides both hospital and community based clinics to assist patients in achieving weight reduction and weight maintenance, prior to bariatric surgery.
Q. How can I be referred to a dietitian?
A. Individuals can access the services of a dietitian through their GP or hospital consultant. The doctor will complete a referral form or letter providing relevant medical information. This helps the dietitian to complete a detailed dietary assessment and provide appropriate dietary advice.
Q. What to expect and bring to first appointment
The initial appointment with the dietitian will last approximately 30 minutes. During this appointment the dietitian will take a thorough diet history and ask questions regarding any past medical history and current medication. This will enable the dietitian to provide appropriate dietary advice. To assist the dietitian, we ask that all patients bring an up to date record of their current prescribed medications and any relevant medical information.
Follow up:
If a follow up is required an appointment will be made for approximately 15 minutes.
Q. What if I cannot make my appointment?
A. If the appointment time is inconvenient or you are unable to make the appointment date, please contact us as soon as possible and we will rearrange this for you.
