Relative Satisfaction Survey

The Critical Care Unit is undertaking a Relative Satisfaction Survey to help us evaluate and improve the way we deal with the visitors that come to visit our patients. This is an anonymous survey.

Every visitor can complete a form, the survey is not restricted to one form per family. Paper copies of this survey are available in the Relatives Room on the Critical Care Unit.

A summary of the results from the forms will be displayed in the Critical Care Unit.

Is the hospital clearly signposted so that you could find your way to the Critical Care Unit? Required

The first time you visited Critical Care Unit, how long after your arrival were you greeted by a member of staff? Required

On that first visit, If you could not see your relative immediately you arrived, were you given an explanation as to why not? Required

During your relatives stay on Critical Care Unit did the nurse caring for your relative introduce themselves to you every time you visited? Required
